I bought a 2002 Honda Accord V6 with 215,000 miles, and the owner said they thought the timing belt was changed around 180,000. I didn't trust that, partly because the tensioner that is the only visible component was ORIGINAL. So I changed it out. But it looks like what they told me was pretty accurate - it looks by the wear like it has not over 40,000 miles or so. The water pump looks good, and so do the bearings, but I would NOT reuse that tensioner, even though they hold up well.
If you're doing the work to get in there, just get a new one. The belt itself says HONDA - it's still a bit legible, and the timing marks I put on it to match to my new one are still on there which may be useful. I'm also including the spark plugs I changed out when I did the belt, because on the chance you can use them I'd rather them get used than just thrown away.
